The district nursing team is attached to the GP practice.

They provide holistic, patient centred care to a range of patients over the age of 18 years who are housebound either permanently or temporarily.

A large part of the district nursing role is facilitating care through specialist services and arranging intermediate care provision to prevent hospital admission whilst providing carer support and acting as a patient advocate.
The team provide high quality patient centred palliative and end of life care, providing psychological, emotional and social support for patients and relatives and referring to specialist services when needed including bereavement support.

The district nursing team provide long term condition management, wound care, leg ulceration care, catheter care, stoma care and continence care. District nurses also asses the requirement of household aids which aim to support independence and home care.
