Registering With This Practice

The practice welcomes new patients. We accept registrations if you live within the practice boundary. An outline of this is found on this link. We would encourage you to register as soon as you arrive in the area and not leave it until you are unwell as records from your previous practice may take some time to be transferred. Please ask our receptionist if you are unsure.

 

To register with this practice you will be required to provide proof of address and complete your medical card or registration form. You will be asked to provide information about your previous address as well as the name and address of your previous doctor. At registration you will also be asked to complete a confidential health questionnaire. This will provide the doctor with important information about your health. We offer all new patients a health check up with our nursing assistant.

When you have registered succesfully, we will send you a confirmation letter inviting you to attend for a new patient health check .

 

NB: Please let us know your new address when you move house. You may need to register with another practice if your new address is outside our practice area.
